Permission to Use an Insect Repellant Spray

My child, ____________________, may have Bug Repellant applied to exposed skin areas before going outside on warm or humid days.

I will provide an insect repellant spray; containing the active ingredient DEET (the active ingredient Piraridin is not recommended). Piraridin is harmful if swallowed and can cause skin irritation for some children.

I will mark my child’s name on his/her insect repellant spray PLASTIC container with a permanent marker.
